Fixed first-run setup blocking playback on macOS when the SubMiner mpv plugin was already installed at the canonical ~/.config/mpv path. Fixed launcher setup gating so stale cancelled setup state no longer prevents playback when the canonical mpv plugin entrypoint already exists.
